Probably due to the fact that Kurt was a fish.
"She looks at me," meaning that they observed.
"Like a fish" in the sense that Kurt was a fish, all are charactreistics associated with this sign, it represents, and they cared.
"When I am weak," Kurt was suffering from a severe addiction to heroin at the time, and was weakened.

Cobain lyrics is always strange. They were so personal that I do not think anyone can ever really know. Nirvana is awesome though.
